Structuring Data for Binomial Logistic Regression with Two-Level Questions

Hello all,
I plan to conduct a binomial logistic regression analysis. The participants completed questions at two levels. The covariates include scores, and the dependent variable is the levels. Could I arrange the data so that one participant is represented by two rows, each for a different level? Please see the attached picture for reference.

If you have repeated measure, you need a logistic mixed model, underground generalized mixed models
